Nerve stimulation at the skin activates two brainstem areas: the periaqueductal grey and the rostral ventromedial medulla. This is known as the PAG–RVM pathway. It sends signals back down the spinal cord to reduce incoming pain. Sleep loss turns off this same descending system.
